Follow this runbook to create an Azure App Service.
Create App Service
In the top search bar type app services.
Select App Services under Services.
To create an App Service, select + Create.
Basics
Project Details
Choose a Subscription.
Choose a Resource group.
Use the Create new link to create a new resource group.
Instance Details
Set the Name.
Info
The recommended naming convention is:
app-<project, app or service>-<environment>-<###>.azurewebsites.net
Set Publish to Code.
Choose Runtime stack.
For static html sites, use PHP.
Choose your Operating System.
Choose your preferred Region.
Different regions influence cost and feature availability, such as zone redundancy.
Pricing Plans
Leave your (OS) Plan (Region) as default.
Choose a Pricing plan.
Click on the Explore pricing plans link to review specifications and costs.
Zone Redundancy
Enable or disable Zone Redundancy if the region you choose supports it.
Select Next : Deployment >.
Deployment

Therefore set Continuous deployment to Disable.
Select Next : Networking >.
Networking
Set Enable public access to On or Off.
Set Enable network injection to Off.
Select Next : Monitoring >.
Monitoring
Application Insights
Set Application Insights to No or Yes.
Depending on previous selections, No may be the only option.
Select Next : Tags >.
Tags
Enter tag Names & Values.
Select Next : Review + create >.
Review + create
Review the summary information.
Select Create.
Overview
The deployment should take ~ 1 minute.
Once complete, select Go to resource to take you to the Overview page for the App Service.
